Complex Needs Floating Support

We reach out to our most vulnerable Adults through our floating support services.

At Circle of care, we put you (the Person living with mental health challenges) at the centre of the service we provide.  We aim to provide high standards of support services that is focused on your unique circumstances and needs to give you the flexibility, choice and control over how we support you to live an independence life with hope.

Our dedicated Support Staff will work with you and the people close to you including Mental Health Care Managers and other Health Care Professionals who understand your needs. This is to enable you to stabilise your mental state and minimise your risk of relapsing.

We will support you to make maximum use of your community/local resources including resources available from independent and voluntary sectors to plan your recovery.

Your Recovery

We aim to support you to live life full of hope with support from our experienced and dedicated Support team. Our approach will be to support you to look beyond your Mental Health illness and encourage you to engage with the community, pursue your interests and live a meaningful life.

Recovery emphasises that, while you may not have full control over your symptoms, you can have full control over your life. Recovery is not about ‘getting rid’ of your mental health problems, It is about seeing beyond your symptom and recognising and fostering your abilities, interests and dreams. * Source MHPF.

Meeting with you 

We will contact you and arrange to meet with you at your convenient location after we have received a referral for you from the Community Mental Health team, we will meet with you and your care coordinator for a formal introduction during which we will talk about you and how we can best support you to achieve your goals.

During this meeting we will discuss your needs, we will draw up your support plan with you, focusing on what you wish to achieve to enable you to live a meaningful and independent life.  Your support plan will focus on your needs and how we can support you to meet those needs. We take your strength and abilities into consideration when drawing up your support plan and work at your pace. The plan will state all the outcomes you wish to achieve, we will review your outcomes every 3 months to see where you are, what outcomes you have met/achieved and what further expectation are to be met.

  • Management of finances such as bill payments, benefits and banking
  • Tenancy and housing related issues
  • Community networks and personal safety
  • Communication, relationships and emotional wellbeing
  • Education, learning and employment
  • Friendships, social life and holidays
  • Personal care, cooking, cleaning and shopping
  • Health, medical appointments and medication
  • Cultural and religious interests and needs
  • Person centred planning and circles of support

Manage Drug & Alcohol Misuse

  • Refer to specialist agencies if required
  • Support to liaise with drug agencies and advice with harm reduction
  • Advice on tenancy issues
  • Support to structure activity for the week
  • Support with relationship problems
  • Advice on financial matters
  • Support with detoxification programmes
  • Support you to join support groups
  • Raise awareness of legal matters
  • Support to engage with the probation & criminal justice service
